In the Subject field, enter: “Withdrawal from the contract – [order number]”.
In the Message field, enter: “I hereby give notice that I withdraw from the contract concerning: [product name], date of the contract: [date]. Email address used for the order: [email]”. If you paid by a method other than bank transfer and would like the refund sent to a bank account, please add the account number.

Check the box “Send a copy to yourself” to receive a copy of your declaration by email. I will promptly send you an acknowledgment of receipt (with the date and time) by email.
